The hormone therapy first year follow-up checklist
A stage by stage checklist for carrying a menopause patient from the initiation visit through month twelve without losing the record that justifies each dose.
Early discontinuation is rarely a decision the patient announces. She stops the patch in week seven because of a bleeding episode nobody chased, or because the night sweats improved just enough that the cost stopped feeling worth it, and you find out at a visit she does not book. Almost every one of those exits has a follow-up gap behind it.
This checklist maps the first twelve months of hormone therapy into four stages and gives each one a small set of actions with a standard for what good looks like. Work it as written for one cohort of new starts and you will see which stage your practice is actually losing patients in.
Before the initiation visit
At the initiation visit
Weeks two to twelve, the titration window
Months four to twelve, maintenance and the record
How to use it
Use it on the next ten patients you start rather than on the whole panel. Ten is enough to expose which stage your clinic actually leaks in, and small enough that you can run it manually before deciding whether the scoring should be automated.
Most practices find the same two failures. The baseline exists but is not comparable to anything later, and the week six check-in is the one nobody chases. Fixing only those two moves the first year continuation rate more than any change to the prescribing itself.

Questions about this document
Which symptom instrument does this checklist assume?
It works with any established menopause instrument, as long as you keep the same one for the same patient across her first year. The checklist cares about interval, wording stability, and where the score is stored, rather than about which validated scale you prefer.
Can I run this without software?
Yes, and running it manually for one cohort is a good idea. The manual version costs a few hours a month in sending and chasing, which is exactly the number the follow-up load calculator estimates before you decide to automate it.
Does the checklist cover compounded prescribing?
The stages apply regardless of what you prescribe, because the follow-up structure does not change. What changes is the counseling and sourcing documentation in the first two stages, which sits alongside this checklist rather than inside it.
After ten hormone therapy starts on paper
Work the four stages by hand on the next ten women you start and you will know which stage your clinic leaks in, and it is usually the week six check-in nobody chased. That is the moment to look at PauseNotes, because the checklist becomes a screen that already holds her baseline, her subscales and every dated dose change beside them. Book a walk through and bring the stage that failed you.
